INSTRUCTIONS:
If you are not able to attend an "Mano a Mano"meeting but would like for another member to cast your votes, complete this form and send it to the "Mano a Mano"Secretary by either (1) an attached e-mail file or (2) printing out and mailing it to the "Mano a Mano" secretary. For this proxy form to be valid, both you and the person you choose to represent you must be members in good standing of Mano a Mano. One person can proxy for a maximum of 2 people.

QUORUM:

Twenty percent (20%) of the membership eligible to vote shall constitute a quorum for the purpose of holding the regular annual meeting or a special meeting of the "Mano a Mano".

PROXIES:

Proxies may be assigned and carried by voting members only. They must be in written form and postmarked to the "Mano a Mano"Secretary thirty (30) days prior to the date of the membership meeting. Members attending said meetings but having to leave early may assign only their proxy to another voting member without previous proxy registration. Proxies in excess of three percent (3%) of the voting membership will be reassigned to other voting members. No other proxies will be reassigned.
